United Medical Centers welcomes patients with or without insurance, regardless of your ability to pay. For those who are not eligible for other forms of coverage, UMC offers a sliding fee schedule payment system. This system is based on your family's annual income and size.

Once you have scheduled and appointment with one of our Eligibility Offices please bring the following required information.
1. Most recent Income Tax Form. If you are self employed bring your 1040 & W2 form or two most recent check stubs, include any unemployment benefits, child support, Temporary Assistance For Needy Families TANF, or any other type of income.
2. Proof of Government Income, AFDC Reports, Pension Reports, Annual Report of Social Security Benefits or Supplemental Benefits.
3. Picture ID's
Bring all required information at the time you are applying for the discounted sliding fee schedule. You may be required to provide proof of residency for other state-funded
programs, e.g., Electricity, Water, Gas, or Rent receipts. *Note: Your physical address is required to be on all the utility
