Under the supervision of the Patient Educator and Clinical Nursing Coordinator, the CCHW is responsible for promoting the “Health Education” program in the Tri-County area and for assisting the Patient Educator during the preparation and conduction of classes, campaigns, and outreach activities. The CCHW serves as a health motivator offering patients in the program encouragement and support, influences patients in the program to make lifestyle changes regarding diet and exercise, and performs various clerical functions ensuring the proper completion and handling of all required forms for tracking all program participation and attendance. The CCHW is also responsible for compliance with other requirements affecting the clinic as State/Federal regulations, TJC Standards, DSHS, and Compliance/ Performance Improvement Standards.
Must have a high school diploma or equivalent and be certified from an approved NRCMA Program.
1. Must have at least one year experience in teaching health-related topics
2. Prior experience in patient education primarily in diabetes, hypertension, and other community-sponsored health education programs preferred.
1. Ability to drive a vehicle and travel as required.
2. Bilingual (English/Spanish) preferred. Ability to communicate proficiently with individuals of diverse backgrounds and economic levels.
